Who’s to blame when something goes wrong?

Who is to blame if something goes wrong or there is an accident when using powered access equipment?
According to IPAF’s research, the fault can often be traced back to the planning stage. Things frequently start to go wrong before work even starts! IPAF’s Plan Ahead for Safety campaign outlines all the elements in a proper planning process, from selecting the correct machines and appointing trained and familiarised operators and supervisors, to conducting a thorough site risk assessment and devising and communicating a proper rescue plan. All operators or supervisors should be prepared to challenge those who planned the operations if something doesn’t seem right, or corners have been cut during the planning phase.
